Those starting to attend football training must provide the coach with a health certificate from the Šiauliai Sports Medical Center (Address: S. Daukanto St. 90, Šiauliai) or a family doctor. Also the application – consent for admission to the football academy “Šiauliai” and the contract for non-formal children’s education services. Medical certificate form, no.:027-1/a. Entry in the medical certificate: “You can participate in football training and competitions.”

The certificate is valid for half a year.
